REQUIREMENTS:
Submission of
Assignments
Palliative care work-up of one patient,
Short essay describing your personal experience with your home
visit, and
Hospice Activity Summary form (from hospice nurse)
Completed take-home examination
Mock Interdisciplinary Team (IDT) Conference:
Participate in a mock IDT conference to include presenting your palliative care plan
Faculty role models will be present, including Hospice Medical Director, Hospice Nurse, Social Worker, Chaplain, and others...
EVALUATIONS:
Turn in completed
take-home examination derived from the
lectures and readings.
Complete evaluation questionnaires of your hospice home visits and the
mock IDT conference.
CRITERIA FOR EVALUATING STUDENT IDT PRESENTATIONS:
Clearly presents in a logical manner the clinical data in the allotted time.
Clinical data is pertinent to the situation
Describes how the expertise of one or more interdisciplinary team members (nurse, nursing aids, social worker, chaplain) was (or might be) used to address selected biopsychosocial problems.
Identifies actual or potential ethical/legal dilemmas in providing
hospice care to the patient and family.
Discusses the role of the primary care physician (not the hospice medical
director) in the care of the- hospice patient and family.
Describes the impact of the terminal illness on the patient and family in
the hospice setting.
